Planes getting ready to take off from landing, where the crash happened. The underlying point here is mistakes made by the pilots, including overreliance on Automated Systems they didnt really understand, leading to confusion and a response that came too late, resulting in three people dying, another almost 200 injured. This was the first deadly crash involving a 777. The ntsb concluding that the probable cause involved overall the flight crew seasoned with a good Safety Record mismanaging the airplanes descent during the final approach, the complexity of the auto pilot and throttle systems, both insufficient description in boeings manuals and asianas training. The ntsb put together 30 findings altogether. Most of them focusing on the flight crew. The three pilots had more than one chance to correct what was a bad final approach into sfo and failed to recognize that, calling for a goaround too late. Experts testifying the pilots made up to 30 different mistakes. And the ntsb concluding it was likely fatigue, lack of communication, lack of training with Automated Systems in the 777 that led to this crash and that asiana needs to give more training to the pilots. Firefighters saved five people, who had been trapped inside the burning plane, but the arrival Incident Commander was not trained in aircraft fires. The overall triage, they said, was effective, except, of course, for the glaring oversight of not seeing or treating that 16yearold chinese girl who was later ran over by two fire rigs. The airports also lacking certain components in its drills, which delayed the arrival of backboards to treat seriously injured passengers, communication among different fire units was also broken. That has been remedied. But communication problems between the airport and city dispatch have yet to be fixed according to federal investigators. Most fundamental course of protection for passengers, though, the investigators are saying, seat belts. The ntsb says the two passengers in 41e and 41b would have been alive, but they were not wearing seat belts. They were thrown out of the back of the plane. The third person who died was in her seat and likely wearing a seat belt, but a door broke during the crash and hit her on final impact. We heard some more details on that. Sfo has taken steps, as has asiana, ahead of todays recommendations and the boards findings and voting to approve them. But before and after the hearing, the ntsbs acting chairman stressed something very important. He said, it seems that technology is Getting Better and that there may be overreliance on these automated functions, and that people still need to be controlled. He said, the bottom line is these new Automated Systems can be opening the door for more errors instead of improving safety, and really summed it up by saying pilots still need to be alert. Officers are on the hunt for three armed men who tased a woman during a Home Invasion robbery. It happened around 4 30 this morning at the del Coronado Apartments on camden avenue. Police say three men forced their way into an apartment and demanded money. One of them hit a man in the apartment with a hammer, and used a taser on the woman. Both of them are expected to be okay. New details this morning on audries law. The new highprofile bill that could change Sexual Assault laws has passed a key vote, but not without some changes. Less than an hour ago, a state Senate Committee approved a compromised version of audries law. The original version required a minimum twoyear sentence for minors convicted of sexually assaulting someone whos developmentally disabled or unconscious. Todays version, instead, requires them only to undergo mandatory sex offender treatment. Jail sentences could still be handed down, but that would be up to the judge in the case. It would not be mandated. The bill was named for audrie potts, a Saratoga High School sophomore who was assaulted at a party in 2012 by her own classmates, while unconscious. She later took her own life. Her attackers spent only days behind bars. Audries law now moves on to the state assembly. New at 11 00, San Francisco detectives are releasing these surveillance pictures in hopes of catching the person they say is an arsonist. They believe this man set fire to the south market area over the past week. Several of the fires were set at construction sites, occupied buildings, and dumpsters. If you recognize this man, you are urged to called sfpd. Secretary of state john kerry spent his second day in iraq, meeting with kurdish leaders. Kerry is pressing iraqs Prime Minister to form a government representative of all iraqis. Nbcs Ayman Mohyeldin reports. Reporter u. S. One of the issues that secretary of state kerry was pressing on was that he wanted the kurdish government or at least kurdish representatives to participate in a new government being formed by Prime Minister nuri al maliki. In recent days, the Security Cooperation between the forces here in the kurdish area have been really critical to helping to draw a line with respect to isil. In fact, the regions Prime Minister has said its time for him to step aside and allow a new leadership to emerge. But theres no indication that is going to happen anytime soon. The u. S. Wants to try to get a political process underway, that forms a new government, that also represents iraqs minority and is not particularly dominated simply by the shia block that is headed the by Prime Minister nuri al maliki. But on the ground, the situation remains, rather tense. The iraqi army says it still holds that the Largest Oil Refinery in the country, despite the fact that isis militants are claiming to have overrun it and are now in control. There are some mixed reports of who is firmly in control of that refinery. The Security Guard accused of attacking a boy in a wheelchair says he was asking in selfdefense. Marshal mitchell is the Security Guard in this surveillance video. Hes accused of handcuffing and punching and dumping the teen from his wheelchair at the school last month. Mitchells Attorney Says the teen involved has a history of disciplinary problems and had spit and antagonized mitchell before the incident. The lawyer also says the student was not 100 wheelchair bound. Santa clara county sheriffs deputies say theyve caught a thief who has stolen a computer. Deputies have charged ryan parraadiso with robbery and possession of stolen property. Last week, he apparently tried to use one of those computers to make an online purchase. Deputies tracked it to his home, where they say they found other stolen goods, including silverware and money. Today, Santa Clara County supervisors could keep more pot shops from opening up. I wouldnt go that far, but keep in mind that cereal is a processed food and Health Guidelines usually recommend that we limit our intake of that, no matter how old we are. But this report says Companies Need to calculate the nutrients added to cereal differently when it comes to kids bodies. Kids love them, those cereals that market to little ones, using fun characters on the box. But a new report by the Environmental Working Group warns millions of children are potentially eating unhealthy amounts of vitamin a, zinc, and niacin, because theyre added into cereal according to calculations for adult bodies. If they eat lots of cereal, this could potentially be a problem. Reporter the report says too much zinc can lead to negative effects on red and white blood cells. An overdose of niacin can cause rashes and nausea. Vitamin a in large quantities is the most serious, says a Silicon Valley pediatrician. Vitamin a is a fat soluble vitamin, so the two can have toxicity. You can have swelling within the brain and there can be liver problems. Reporter he says your children having a childsized bowl of cereal is nothing to worry about, but cutting that down to three days a week and adding fresher foods would be better. It brings up the issues of good eating habits for children. Children shouldnt be fed cereal all day long. They need various things in their diet, not limited just to cereal. Reporter kellogg told usa today that the report ignores a great deal of Nutrition Science and consumption data showing that without fortification of foods such as readytoeat cereals, many kids would not get enough vitamins and minerals in their diet. A reprieve for local students attending three colleges under investigation by the u. S. Department. The u. S. Department of education has cut a deal with corinthians colleges, which owns heald, everest, and wild tech colleges. Theyll get federal funds to stay in operation until current students can graduate. Corinthians is under investigation for falsifying job placement rates, altering grades and attendance. The biggest water provider in the bay area says it will not call for mandatory water rationing this summer. The San Francisco Public Utilities Commission Says because people in the city have been doing a good job of conserving water already, it does not have to require water rationing. The water system provides water for nearly 3 Million People in the bay area. It is only at 65 capacity. But leaders say that does not mean people should stop conserving. In fact, the city launched a provocative new campaign yesterday, with ads, one of them saying, customers should make it a quickie in the shower. The ads are meant to get peoples attention, and the other says jiggling the handle of a leaky toilet will not fix the problem and you should replace your old toilet and something about dirty hands as well. Starting today, your daily cup of joe at starbucks could cost you more. The company says its raising prices for some of its drinks by 5 to 20 cents. The price will vary depending on where you live. Well, it has everything to do with our wind direction. So, all the cities that have the winds coming off the ocean, its actually still pretty comfortable. Look at whats going on in santa rosa. That wind direction shifted from the south to the north, and as a result, you jumped by 10 degrees over the course of an hour. Now, once that wind kind of backs off a little bit and shifts more westerly, we are going to see those temperatures drop back down. But its going to be warm out there and the wind is the weather story of the day. You can sigh in San Francisco, old glory getting quite a bit of pickup.
