The department will also establish a single regional structure that brings together a wide range of services, including health care, loan centers and other benefits. The goal is to streamline whats being called a clunky, confusing system that required multiple user names and passwords. These reforms are the results of the embarrassing scandal that rocked the va earlier this year. Reports that at dozens of facilities around the nation, veterans were waiting months for treatment and records were manipulated to hide the problems. Jim, thank you. The lava flow in hawaii has claimed its first house. The molten lock flowed underneath the home igniting the wooden blocks and the rest of the structure. The lava started in june. Its traveling about 3 to 5 yards an hour so the renters of this home had plenty of time to evacuate. Families in floridas socalled sinkhole alley also evacuating. Take a look at that. This sinkhole swallowed a car and most of a homes driveway in just 15 minutes. Five other families nearby were also told to get out. More than 250 sinkholes were reported in the Sunshine State last year. A third of them in just three counties. This is my hometown, sinkhole alley. A report on the Yorkville Endoscopy clinic finding the facility failed to follow a number of procedures. The 81yearold comedienne was ever undergoing a scheduled procedure for acid reflux in august when she went into cardiac arrest. The report by the department of Health Claims the clinic failed to identify deteriorating vital signs and provide timely intervention. Rivers was sedated for the procedure with propofol, the same powerful anesthetic involved in Michael Jacksons death. The report saying her dose of propofol was inconsistently documented. The highly critical report goes on to say rivers didnt consent to every procedure that was performed, including one conducted by her surgeon who wasnt even authorized to work at the clinic. And that a staff member proceeded to take pictures of rivers with his cell phone while she was sedated. Now attorneys for rivers daughter melissa have released a statement saying she is outraged by the misconduct and mismanagement now shown to have occurred.
