That drug wouldnt have killed the bugs that typically cause ear infections. This finding worried the cqc, the watchdog for online prescribing services, as a result they inspected hr healthcare, who run treatit. Com, and another online provider, md direct. The reports published today highlight concerns with both firms over safety, inappropriate prescriptions, and the qualifications of clinicians. Hr healthcare told us they are now working to improve their microprocessors and the error with the medication we received was due to a system glitch. Following our inspection, md direct voluntarily cancelled its registration. The cqc has so far inspected a quarter of the companies registered in england. Weve now inspected 11 of these providers. The first two reports are published today, and they are shocking. Look, i really understand the difficulties the public have because the websites look incredibly professional, but the registration with the Care Quality Commission is really important, and we are now inspecting all of those sites. And theres another concern, the sale of antibiotics on the sites. Because bacteria become resistant, standard treatments become ineffective. This is a particular worry to lord 0neill, who led The Governments review on how and when antibiotics are used. We need to stop treating antibiotics as though they are sweets. And we need to be a lot tougher and disciplined about how they are being used. Our review showed if we carry on down the same path around the world, in another 30 odd years 10 Million People a year will be dying. It is one of the worlds most famous institutions, aimed for school parties, historians, families, or simply the curious. Conflict, shocks and fascinates us, this story began a few miles from here with a Grand Ceremony in a cathedral of glass. By the time george v opened the Imperial War Museum in its first home at the ill fated crystal palace, he already had plenty to see. The museum had begun collecting exhibits while the battle of the First World War was still raging. From the outset, the founders set out to involve the public. Appeals printed in ration books brought a flood of personal memorabilia. The idea was initially that it was to be firstly a sort of memorial to the war, but primarily it was to cover peoples experiences in the war, and that meant it covered everybody from the front line soldier to the munition Factory Worker to a civilian. The best way to illustrate how the Imperial War Museums have changed since the first collection was put together a century ago is to look at what they hold today. There are 155,000 exhibits, those are the larger items either on display or held in store, but there are well over 33 million documents, photographs and recordings, all of them helping to tell the stories around those exhibits. Being charged with those horrific crimes, that wasnt my character as a person, but, individually, you know, my family, as well as my friends, they knew i was innocent. When you went to jail, what State Of Mind were you in . Were you determined to fight . Theres no way in the world i can stay in prison for something i didnt do. So. Fighting it to that extent, you know, it never came to the equation, because i still, you know, had a sense of hope, thinking that, you know, individually these charges would be dropped and i would be free. When you went to jail, you could barely read, during yourtime injail you passed your High School Diploma and you studied law so that you could become your own advocate. When you had that focus and that reason study, you then find yourself a natural student . Yes, i mean, i had no luxury to quit. I was sentenced to life in prison, and life in louisiana is, like, Life Without Parole or suspended sentence, you know, i was sent there to die. I had no luxury to quit. I mean, i had to, you know, i was compelled, because i wanted to be free. I stayed on the fact that i knew the truth, i was innocent, and justice will prevail, i did not have a lot of choice. Can you believe you are out now . Oh, yes, yes i do. Lets bring in your lawyers, because these were the two people you got in touch with at the Innocence Project in new orleans. And what they went on to discover was mind blowing in terms of the Miscarriage Ofjustice you had been a victim of. And the fact you had been framed for crimes you did not commit, as we said. Roberts case presents every single thing that can go wrong in a criminal case in an overcrowded courthouse, in a place that looks up more young black men than any other place per capita in the world. The judge, the court system failed robert, the District Attorneys Office failed robert, and his lawyer failed robert. And whether some of those failings were deliberate, sorry, whether they were a product of an overburdened system, or whether when you get down to it there is certainly some deliberate misconduct on part of the state, knowing that they had prosecuted another man for this, and charged robert with it anyway. Those are the kinds of questions that are going to be answered over the next couple of years in professional liability issues with some of the lawyers who were involved. During the years that you were injail you had three children who grew up without you being able to be there as a father. You can never get back the experiences that you would have had during that part of your life. How do you come to terms with that . Because im free now, im nota monster, im a better person in the sense of understanding, you know, whats going on in my society, posed to when i came and when i went to prison. I was a young, confused boy, didnt understand what was going on, but now i do. My kids have done well. The system didnt destroy me, it didnt destroy my kids, were moving on, and that dont make it right, because what happened was wrong, it was an injustice. He was flying too low and too slow when he started a manoeuvre which led to the deaths of 11 people. We have just had the final safety reported to this accident, more details and a couple of minutes. The final report into the shoreham Airshow Disaster in which eleven people were killed has concluded that the pilot of a Vintage Plane which crashed could have taken action to abort his display. Air Accident Investigators say the Aerobatic Manoeuvre started too low and too slow, the pilot didnt realise how low he was and there could have been an issue with being able to read the flight instruments. Transport correspondent Richard Westcott is at Farnborough Airport for us with more details. You can see how big and comprehensive it is, 18 months worth of work, but i think what you have just said sums it up. Effectively the pilot went into this Loop Manoeuvre, going to slowly, too low as well, perhaps 400 feet lower than you would have expected him to be, when he got to the top of the loop he was in 2700 feet, he would rather be a 3500 feet normally doing that. They found the engines were not on full thrust at the top of the loop, which would normally be the case. The big question is why . Was the pilot confused or were there other circumstances . That is a little bit more grey. They know the pilot could have pulled out of this loop but did not, they dont really know why because they interviewed him several times but he cant remember the accident. There has been an issue with the engines on this type of plane and other circumstances where they have suddenly dropped power and nobody has been able to work out why. They cant work out or conclude whether the pilot didnt put the thrust on enough, didnt have the engine going fast enough, or whether it was a fault with the aircraft. Here is my colleague, duncan kennedy. This was the Hawker Hunter jet at the centre of the shoreham air crash. Of of Vintage Aircraft that would later disintegrate crowded road. This is the jet over the shore area the day of the accident. During one manoeuvre, this happened. Thejet crashed by da 27 but run alongside the showground. Oh, my cobb oh, no. The fireball engulfed drivers and bystanders, a disaster measured in seconds that gave little chance of escape. Today, error Accident Investigators produced this video of what they believe happened. They say the pilot was around 300 feet lower than he should have been at the start of the manoeuvre, there was not enough Engine Thrust or speed during the loop and the aircraft was too low to perform it. It may be the pilot misread his height instruments. He was not trained to escape the accident in the Hawker Hunter and hit the ground. 11 men were killed, it was the worst as Show Accident in britain in more than 60 years. 0ut show incident. Among those who died were two friends, Matthew Greenstone and jacob schilt, both 23, and both players for worthing united football club. The club vice chairman, mark sanderson, says the disaster continues to haunt all of those who knew the two young men. And it should never have happened. We wear their names on our kit. So theyll always be remembered by us as great footballers. First of all, they were superb players. And also brilliant friends. The pilot of the jet was andy hill, he survived the crash with serious injuries. He has no memory of the crush. Todays report suggests he may have confused his speed and altitude with other aircraft he had flown. The report found deficiencies with organisation and maintenance as well. The aircraft was not compliant with its pilot to fly. There was a lack of clarity over who at the airshow was responsible for safety and controls to protect the public we re and controls to protect the public were ineffective. The Civil Aviation authority which governs safety at British Air Displays has brought in a number of changes following this air crush. It has increased the distance between the display of the crowds watching below, and it requires pilots to be Better Qualified before carrying out these acrobatic manoeuvres. That may give some measure of understanding and comfort to the relatives and friends of those who died. Some other details that duncan alluded to, it paints a very confused picture of who was in charge of safety at that airshow, was that the organisers, they thought the Safety Regulatory had overall charge and the Safety Regulators thought the organisers we re regulators thought the organisers were on top of things. No one, critically, was looking at or thinking about the safety of people who were not at the airshow that happened to be driving past or walking past on the a27, those are people that lost lives. Critically, they were not thinking of something goes wrong outside of the area of the airshow, what happens . This report suggests it would not have been nearly such a Syriza Accident of somebody had paid attention to those elements. Nearly such a serious accident. They got a test pilot to fly another aircraft in a similar look to find out what pilot andy hill could have done, even four microseconds after he had gone over the top of the loop it was still possible to rescue the manoeuvre, there was still a way to fly out of this problem. Even fall seconds. He had not had the training. All critical things that potentially, who can say, potentially could have prevented the accident or meant that not so many people lost their lives because the aircraft would not have been flying over a busy road in the first place. The reports published today highlight concerns with both firms over safety, inappropriate prescriptions, and the qualifications of clinicians. Hr healthcare told us they are now working to improve their microprocessors and the error with the medication we received was due to a system glitch. Following our inspection, md direct voluntarily cancelled its registration. The cqc has so far inspected a quarter of the companies registered in england. Weve now inspected 11 of these providers. The first two reports are published today, and they are shocking. Look, i really understand the difficulties the public have because the websites look incredibly professional, but the registration with the Care Quality Commission is really important, and we are now inspecting all of those sites. And theres another concern, the sale of antibiotics on the sites. Because bacteria become resistant, standard treatments become ineffective. Fighting it to that extent, you know, it never came to the equation, because i still, you know, had a sense of hope, thinking that, you know, individually these charges would be dropped and i would be free. When you went to jail, you could barely read, during yourtime injail you passed your High School Diploma and you studied law so that you could become your own advocate. When you had that focus and that reason study, did you then find yourself a natural student . Yes, i mean, i had no luxury to quit. I was sentenced to life in prison, and life in louisiana is, like, Life Without Parole or suspended sentence, you know, i was sent there to die. I had no luxury to quit. I mean, i had to, you know, i was compelled, because i wanted to be free. I stayed on the fact that i knew the truth, i was innocent, and justice will prevail, i did not have a lot of choice. Can you believe you are out now . Oh, yes, yes i do. Lets bring in your lawyers, because these were the two people you got in touch with at the Innocence Project in new orleans.
