First of all, the way in which coronavirus has been spread, you have done some work on the fact that you think the majority of cases from china may have gone undetected. Talk to us about that. It is not the majority
of cases in china per is he but looking at the cases among travellers that have moved elsewhere and that is by comparing the number of cases confirmed in singapore which had a relatively high number of cases given passenger numbers compare to the number of cases that have been detected in other countries. And we used singapore as a benchmark or singapore and a few other countries that also report relatively high numbers. Looking worldwide we estimate only one third of the cases are among travellers have come out of china had been detected. So, if that is true, that leaves the possibility of the potential for chains of transmission out of these undetected cases. You would now, if that is true, naturally expect to see quite a spread, given that these cases had gone undetected . Spread, given that these cases had gone undetected . There is that potential. The is the caveat of course that you are less likely to detect cases that have mild symptoms. If it is the case that people that have mild symptoms also
have lower levels of virus, they are potentially less infectious for a given level of contact. And so, it may be that they are not spreading as much as the severely infected disease cases but it is still worrying and it means there needs to be vigilant across the board in all countries. What makes a person more liable to spread this virus, is it about their own tolerance of the virus . Generally, in diseases, there will be variation between the virus levels between individuals and individuals with higher levels of virus may be more likely to spread. So that is one of the things about the pathogen and possibly the interaction between that persons body. But there could behavioural things, for example if someone is coughing around a lot of people in close contact or if theyjust have a lot of close contact compare to someone lot of close contact compare to someone who is more isolated. It can bea someone who is more isolated. It can be a combination of the virus, the persons behaviour and also their
individual setting. In the case of sars which is of course are related coronavirus, there was one individual and hong kong who used the toilet in tower block that had a fa u lty the toilet in tower block that had a faulty sewage system. That amounted to 199,000 trolley waits in corridors and side rooms. Thats more than double the numbers seen four winters ago. And a growing number of patients experienced delays before they were even seen. One in seven arriving by ambulance waited over 30 minutes before being handed over to hospital staff. I am able to observe these patients and prioritise them and make doctors aware that i think these patients should be seen quicker than what they would normally be if they were left out in the corridorfor any length of time. A Royal College of nursing survey suggests these roles are commonly used now, with nine in ten emergency nurses in england reporting that it exists where they work. It is multiple reasons that have brought us to the side. Emergency ages are often the manifestation of the challenges of the health care system. When we dont generally have enough nursing staff, throughout the nhs, that also applies to social ca re systems nhs, that also applies to social care systems as well. Where we dont have the staff and the resources to look after people and care for people, and the right environment in their own homes, or in other more appropriate environments, that brings people to Emergency Departments because there is no
infrastructure to be looked after in the right places. In the same respect, it also holds up the flow of patients through the Hospital System to go back out to areas where they may need district nurses, community nurses, practice nurses, all these other different areas of nursing to support their environment. Interactive figures show that this winter has been considerably worsened last winter, in terms of finding a place for patients. Why is that . Again, i think it is an ongoing series of problems that have not fully addressed. We know that there are around 43,000 nursing vacancies in the nhs. And this is basically putting an intolerable pressure on nursing throughout the system, but actually, the survey itself really highlights that Emergency Care nurses are absolutely having to deliver care and conditions that no one goes into nursing to have to do. Some of the narrative within the survey when i read it as a nurse myself, it was really upsetting, to hear of how people are not able to wash people in a private area, just with a blind rail around them or feeling that they cant attend to people quickly as quickly as they wa nt to people quickly as quickly as they want to and leaving their nursing shifts, just feeling they have not delivered the care they wanted to. As you know, we have been reporting extensively on the spread of coronavirus. And the Health Secretary was talking in commons today about the fact that there are measures in place to deal with it if there is a significant outbreak. How prepared do you think the nhs is to deal with it . I think the prepared do you think the nhs is to dealwith it . I think the nhs has a lwa ys dealwith it .
