Poor reporting contributed to Shelby County's COVID-19 vaccine waste

Shelby County Health Department Director Alisa Haushalter sent the Tennessee Department of Health a hand-written log of wasted Pfizer COVID-19 vaccine doses on Monday. 
In emails to each other, Tennessee State Health Commissioner Lisa Piercey and Dr. Michelle Fiscus, the head of Tennessee's immunization program, described the log as "chicken scratch." 
Further documents The Commercial Appeal obtained through an Open Records Act request show that while Haushalter — who tendered her resignation Friday — and other Shelby County officials were in frequent contact with the state over the past two weeks, they made no explicit mention of wasted vaccine doses until Haushalter called the state Feb. 19.
