N is the former assistant secretary of defense for nuclear, biological and defense programs at the department of defense. He worked for the Obama Administration and will speak to the creation of a defense response to epidemics and how the Trump Administration has failed. I think the main point i want to over toore i turn it the experts is that when you have a global, infectious crisis , which is endangering people all over the world, and the people of the United States, and in fact, the people of the world who look to the United States for leadership want to know that we have an administration in this country whose decisions and. Omments are based on science not based on tweets that have no scientific basis. Not based on politics, not based on policies designed to protect the wealthy and the powerful. Might, thereif i are many more comments made trump. Y by president President Trump i think is quoted as saying that he has a ability toural understand the coronavirus, and that the virus will end by april. Donald trump does not have a natural ability to understand the coronavirus, and his reckless statements are confusing people in this country and all over the world. Donald trump has congratulated , when inits effort fact the entire World Community understands that because of a lack of transparency in china, this situation is much worse than a chair to have been. Donald trump has said that, if we have thousands or hundreds of metsands of people who get just by sitting around and even going to work, some of them go that. But they get here you have an incredibly Infectious Disease that the whole world is wrestling with how we prevent the spread of that disease. To suggest to people that if you are sick you go to work, that could be the stupidest advice ever made by a president of the United States, and on and on it goes. The point i want to make is that we need an administration whose science,re based on and that means we should not have somebody like Vice President pence heading up the Coronavirus Task force. Let task force should be a 100 nonpolitical Task Force Led by our best scientists and Public Health experts, not politicians. If there is ever a time you dont want to mix politics with a healthcare crisis, this is that moment. , and importantly, we remain, as all of you know, the only major country on earth not to guarantee health care to all absurdity ofnd the that situation becomes clearer. Very day and why we must move toward a medicare for all health care system, so that every person in this country has health care as rightt, what we must do now, today, is to make sure that every person living in this country, right now, including the undocumented, i may say, must be would to go to a doctor or hospital in order to determine their health status. In other words, we dont want anybody who is sick today, saying, i have some symptoms, but i will not go to the doctor because i cant afford it that is horrific for that person and certainly for anybody that person has contact with. The federal government right now must make sure that we are paying all of the costs for those doctor and hospital visits. In the midst of a potential pandemic, no one should be stopping themselves from going to a doctor simply because they are afraid that they cant afford it. At a time when half of the people in this country live paycheck to paycheck, and when people go to work every single day even when they are sick, because they need an income to take care of their families, the United States remains the only major country on earth not to guarantee paid sick leave and paid family leave. In a generald sense, but it is particularly dangerous, given the moment we are at right now. That means that today, there are people going to work who may well have symptoms of the ,oronavirus or other illnesses and in the midst of this crisis, when we dont want people going to work who are sick, who may have the coronavirus, we must guarantee that all people who do not go to work or selfquarantine themselves because they think they made have the virus receive a paycheck to keep their families and their own lives going. Not a radical idea. If we were living in the system we wanted to live, that i want to see, of course, everybody leave. Ave paid medical in the midst of this crisis, i dont want anybody because of the lack of income to go to work and perhaps infect others, because they dont believe they will receive income to take care of their families. Furthermore, it goes without saying that the United States must work with scientists around the world to aggressively for thea vaccine coronavirus. The Trump Administration has suggested, as some of you know, that the vaccine might be too costly for some people to afford. How vulgar and obscene is that idea, but you are rich, you can get the vaccine, you are or, you vaccine. The you are going to die, youre going to live, so that the Drug Companies can make their outrageous profits. It is not acceptable to the American People. When that vaccine is developed, and it must be developed as quickly as we can working with folks all over the world, obviously, it should be made free to every person in this country. Furthermore, right now, there are doctors, you will hear more discussion about this in a moment, doctors and hospitals that do not have the necessary testing kits that they need in order to determine whether or not people have the coronavirus. And there are hospitals all across the country today, even before the coronavirus, who dont have the necessary icu units that they need. That is a condition we must focus on. Every hospital in america must have the proper number of icu officeand every doctors in america must have the testing kits that they need. Furthermore, the federal government must make sure the doctors and nurses and all medical personnel have the knowledge and protective equipment they need in order to remain healthy. We want everybody to remain healthy. Our frontline fighters in this struggle, our doctors and nurses, if they go down because of the illness, who is there to protect the American People . We have to be working overtime to make sure that every doctor, every nurse, every medical professional in the country has the equipment they need to protect themselves and the knowledge as to how they can protect themselves. They United States government clearly must do a better job working with other governments around the world. This is not an american issue, this is a global issue impacting countries in every part of the world, and we have got to be invested in and working with the World Health Organization and other International Health organizations. What i think frightens the American People is that today, we have a president of the United States who does not even recognize the reality of climate understandingne that we have to work with the entire Global Community to address this crisis. Perspectives as a United States senator, of what the government should be doing. Now i want to get to some of the medical professionals and get their specialist perspective on the crisis. Lets begin with dr. Abdul el sayid. Thank you, senator. I will be sparking specifically having been a Health Commissioner. Focused on trying to understand the stages of epidemic response, and where they may have gone wrong and what it would look like if they had gone right. The first base is ascertainment there is something wrong. Our countries focus on this has been framed around a Global Health security agenda, but that agenda was for an out of the ebola crisis but thankfully was marginally relegated to west africa because of the heroism of our u. S. Public Health Service and our c. D. C. Learning from that experience, we decided that we need to make sure we have bases worldwide in 49 Different Countries where we can be looking to surveil. For the past four years in a row , funding has been put on the chopping block. 80 of that funding. Taking us from 49 countries to 10. Thankfully, the leadership of Congress Stop that from happening. But you can imagine working in an institution where every year you are told you are about to get cut. How do you plan longterm . The work that due to you do day to day keeping us safe abroad . Second point is that this emergent pathogen occurred in a time of International Conflict within the u. S. And china in the form of a trade war, which you have to ask, how do you actually work with another sovereign nation when you are hearing in the media all day that these folks are trading jabs over trade. That might have curtailed our ability to engage in that contest. Containment, he is taking work of identifying everybody who has been exposed to the virus and figuring out who they may have exposed and either subjective mental quarantine or isolation depending on their exposure status. But in order to be able to do that well, it takes a lot of human power. What happens when you cant rule out a case among somebody who has been exposed . What happens is you say, three people were exposed, all three people you have to assume have the virus. Which means you have to trade all three of their contacts, which gets 29, then 27 mother going. Keep because Public Health funding has been cut over and over again, you realize that without the ability to say that person is definitely not a case, because we dont have the testing kits to do that, what happens is that level of folks you had to contact exploded quickly, overwhelming our ability to do the basic work of containment. Lastly, we are now at the cusp in a number of, communities where we go from containment focusing on every individual case, to mitigation and control. That means largescale interventions around reducing the amount of administering between people such as reducing the virus between people. The challenge with that as we are all seeinge and that the stock markets are telling us, is that it can be challenging for the society and the economy, because stopping the flow of people is basically stopping the flow of the economy. We are at that point now with these outbreaks especially in Washington State, where we have moved from containment to mitigation. The question becomes now, as it moves throughout the United States, what does it take for us to invest in communities in invest Communities Capacity to contain. What does it mean to have the resources you need to do basic Public Health work, and more important, where has state and local Public Health funding been . We have been a stagnation if not declined since the administration. Where does it mean for us in this position in society where we need to be investing in mitigation efforts which put a damper on our ability to do what do, and what the consequences look like for working people, Small Businesses and entire sectors of the economy . ,e did not have to be here and we should not be here had reinvested in a capacity to take on these emerging Infectious Diseases in the moments they emerged. We would not be here as a society. Under this administration, that has failed. Sen. Sanders thank you. Lets go to the director of the infectious for the disease modeling. [inaudible] legitimizectively liberty. My team and i collaborated closely with the ministry of health. What we observed was that the itial heavyhanded exacerbated transmission because getle are too fearful to treatment. Trust is instrumental to the hospitalization of cases, which in turn played a role in curtailing the epidemics. Unless camila and that people can trust is essential in this crisis. As senator sanders highlighted, covid19 is exposing the systemic frailties in our pandemic prepared has. For the millions of people who are in short or underinsured, the medical bills that could be incurred may be a real deterrent from seeking health care. Compounding the problem, families living paycheck to disproportionately be impacted by the control measures such as school closures. We need to ensure that people have the means to stay at home if they or their children feel unwell. Adequateo not have ventilator and icu capacity to handle the likely growth of the pandemic. According to our epidemiological simulations, it will only be a matter of weeks before critically ill covid19 cases exceed the number of available icu beds. We evaluated a range of scenarios across Different Levels and key parameters. Despite certainties and other factors, the vast majority of scenarios, at the peak of the epidemics, we will need many more thousands of icu beds and ventilators that are currently available. The majority of infections are mild, of course, that is good in a sense, however, the problem is that mild infections can still transmit covid19 to people who may be vulnerable to severe infections, such as the elderly. This makes it challenging to control and makes diagnostic testing paramount. Sen. Sanders thank you very much to dr. Alison galvani. Y is a family practitioner here in michigan, and like many thousands of family practitioners, she is on the front lines and has to deal with this crisis on a daytoday basis. When you look at the symptoms pretty virus, they are nonspecific fever, cough, shortness of breath government running nose. That is pretty much what i see all day. With the flu season ending and it being a bad flu season. What does it mean for my patients . It means that for people with mild symptoms, they dont want to go to the doctor and pay their copay and their deductible. They will just want to stay home and drink more fluids. So those are the obstacles and hurdles of getting to my office because of financial reasons. Myse who do make it to office, currently we have such strict guidelines of who can be tested, meaning you have to have we justposure, wears as discussed, people have mild illness. In michigan, you can only be tested if you are in the icu or you know you have exposed to coronavirus. But how are we supposed to know if the symptoms can be so mild. We are not testing enough people. There is a shortage of testing and it will lead to more people getting sick. We are not testing the amount of people we need to test. The state can say we have x amount of tests on hand, but if you only testing people who obviously have the illness, you know you are not doing enough testing. So the people who are most vulnerable, like People Living with diabetes, they cannot afford their insulin because of the way our Health System is now. You have people who are vulnerable who cant afford to take care of their chronic illnesses. You have People Living with chronic illnesses and they will be the most vulnerable to pick up illnesses like this. You have problems with things masks. I cannot order the masks i need to protect myself and my patients. When doctors and nurses are exposed to mild cases because we are not masktesting people, what happens when we all get sick . Then you have no doctors or nurses to take care of you. I feel like if we have learned anything from this epidemic this will not be the only one in michigan, a few years ago, we had an epidemic of hepatitis a, a virus transmitted through food. Food care workers, because we dont have a livable minimum wage in this country, when they are sick, they cant get to the doctor. The only way to get a hold of that epidemic was by making a free. E and making it if we learned anything from this epidemic, i hope that we learn as a nation that we all deserve health care, we all need health care, and that it should be free at the point of service for everyone. We deserve health care in this country, not just wealthcare. Sen. Sanders thank you very much. Dr. Berger is the president of the Largest Nurses Union in the country. Good afternoon, everyone. Sanderso thank senator for inviting us to come and share our expertise on covid19, and most importantly, for showing leadership in this health crisis. And ie is deborah burger, have been a registered nurse for over 40 years. Nursesesident of the united, the Largest Union and professional association of registered nurses in the country. As you know, nurses are on the front lines of our health care , weem, and at the same time are also scientists who understand well how Infectious Diseases spread. Positioned touely recommend what we need to do at the ground level, in our health care facilities, to contain and stop the spread of covid19. As union nurses, we have the freedom and protection to speak up on behalf of ourselves and our patients. Protect theto health of our patients and the public as well as our nurses and our colleagues. Nurses are eager to take care of patients and make sure that our communities are safe, but we need the right staffing, equipment, supplies, communication, and training to do this safely. Put simply, if we are not protected, our patients are at risk. This became clear that Uc Davis Medical Center in sacramento, resultedcovid patient in the quarantine of 124 hospital workers, including 36 registered nurses. That means that the burden of taking care of all other isients in the hospitals putting a burden on the remaining nurses and other workers in that system. Nurses follow the precautionary principle in our approach to another wayh is of saying, better safe than sorry. It also means that you always take a safer route, where the higher wear a higher level of protection, adopt a stronger standard, even if you dont know for certain that there is something dangerous or not. Particularly important when there is so much we simply dont know about this virus. But this is not the approach of the Trump Administration, the federal, state and local agencies have taken. Recent survey of nurses conducted by us across the board shows that a majority of our Health Facilities are nowhere near prepared to handle this virus safely. They dont have the equipment. They dont have the stockpiles. They dont have the isolation rooms. They dont have the procedures. And they arent communicating with their staff. They have no plans. Of relaxing standards and efforts, which some agencies have urged, we need to be stepping it up. National nurses united has ,etitioned osha and the c. D. C. And many agencies in california, as the epicenter of the disease here in the United States, to do just that. We look forward to helping them implement our regulations. Andame is Gregory Whitten i served for seven years as a special assistant to the secretary for biological, defense and medical programs. I am here to talk about the effort. This disease and pandemic and how we can recreate what we did in the Obama Administration and what senator sanders has been talking about for a long time. The Trump Administration has made us isolated from a lot of the world most specifically the trade battle with china has caused us not to be able to about diseases like these. Mers, this is one of the diseases, but it will not be the last of the diseases that will calm a sure. Virulent,ll get more and we need to be able to work who, and all our allies to make sure everyone is safer. Thank you very much for having me here, senator. Sen. Sanders thanks very much. I wanted to ask the panel some questions, but before i do, i want to pick up on a point the panelists of already made, which on sot how are we behind many basic needs compared to other countries. We are the richest country on earth. In the city of detroit, somebody correct me if i am wrong, in this city, there are tens of thousands of homes where people cannot access water. That their water has been shut off. Can you believe that in the year 2020, in the richest country in major american city, people have no water because they cannot afford to pay their water bills . We are told, everybody up, here, doctors will tell you especially this woman, wash your hands. A little hard to wash your hands and keep clean if you dont have Running Water because you are too poor to pay an outrageously high water bill. In the midst of the water crisis, i understand it, the city of detroit today has stopped the water shutoffs. That is a step forward, and i hope every city in america does the same, because people cant be clean if they dont have Running Water. You an example of how far we are behind the rest of the world in dealing with basic human needs. Let me open the discussion here, and start off with him do, but other people can jump right in. Given the fact that we are not getting good science from the Trump Administration, people would like some kind of expert knowledge about, where do you think things are going to go in the future . Where are we going to be a week from now, two weeks from now, months from now . I know that nobody can predict it with certainty. Whether you start off, then we will go to dr. Galvani. Sticking very close to the evidence, if you look at the studies published out of wuhan, china, where this pathogen originated, we often look at two numbers. R0, thehem looks like a number of cases expected to emerge from one infected case. And the other one is time to read how long does it take between one case turns into two, turns into four. In the wuhan studies, you are looking at 23. If you look at the cluster in Washington State, the doubling time is six, and it can change based on the density of a particular community and how ready the Public Health infrastructure is to observe the pathogen. The double time is six. Right now the latest estimates follow thet if you math, by this time next week, you would expect 1000 and you keep going from there. My hope is that our Public Health infrastructure is strong and that we will start to see a capacity for containment in the communities that havent been hard hit or dont yet have a case because they have the time to ramp up. Also, because hopefully we now will have more of the testing kits needed to support the containment efforts that i discussed earlier. But the challenge here is that we know that this virus does not affect everyone equally. Younger folks or folks without chronic disease, and probability of serious illness is low, probability of death even lower. But among folks who potentially have a chronic illness, the probability is higher. That is why, unfortunately, if you look at the cluster in Washington State, that is the picture. A group of folks who are particularly vulnerable to both a serious illness after acquiring the pathogen, and to death in one community space, where it may pass between them. Those are the kinds of spaces that will be hit the hardest. The consequences, is that this will really stress over health care system. We really need to be thinking about what it means to ramp up early and to anticipate the number of serious illnesses that will require a ventilator, capacity icu, with enough rooms that you are not passing the virus between the rooms. The other thing is that the mitigation protocol that we can to include asng we have seen in Washington State, mandated reductions in congregation, limitations on reducedransit, interstate travel, and potentially closing Public Schools and workplaces. The consequences of that for the economy and, the stock market this morning is showing us, can be dire, and the cost will be far worse amongst folks who are living paycheck to paycheck, because you can imagine, say you are working as a waiter or a waitress at a restaurant, folks are not going out to restaurants in this context. You are living tape to tape, and that is going away you are , and that is tip going away. We have to think about the social economic consequences that will hurt people who, because of the system we live under right now are barely making it as it stands. Sen. Sanders thank you very much. I would like to address the question by talking about cases that will need icu treatment specifically. We have a range of scenarios that we have insinuated with different values that abdul mentioned, and different self isolation, a lot of people will socially,ling less and that will reduce transmission. I do expect the transmission rates will be declining. However, said that, for relatively conservative 2. 5x as abdul said, even higher in Washington State right now, taking into account that only five percent of cases the wuhan data, 5 of cases will need to be hospitalized and 5 of those will need to be treated in the icu, and also taking into account that it is about a based on the wuhan data, we will need almost , in one example of our simulation scenarios. That will be at the peak of the outbreak, which we expect to occur according to that scenario , in the next few months. Compare with what we have in this country, 90,000 icu beds. And about 60 are already occupied at any time with other diseases. Another point also to make is that the spatial distribution of those beds, we need 300,000 perhaps, that is a cross the thinky, but we have to also about the spatial distribution of those beds. We might have a cluster of cases , and we might have availability of icu beds in other locations, but we have to think about how are we going to get the patient to the open icu bed, or what strategies we are going to employ. Sen. Sanders let me go to some of the frontline folks who are here and ask them whether they think they and their colleagues are prepared with what they need. Dooley, what is your office and family practitioners all over the country need right now in order to be better prepared for this crisis . Dr. Dooley absolutely. We need more tests, we need the right as physicians to determine who needs to be tested versus these strict stringent right interior. We need to be able to freely test patients we feel are most at risk. People whoinclude dont have on ovi is known source of exposure of the coronavirus. Dust dont have an obvious known source of exposure to the coronavirus. We need masks. Again, if you are using all the masks and doctors and nurses run out and all of them are six, there is no better to take care of you. They have also done studies of people who wear masks, they are less stringent about hand hygiene. Hand hygiene is key, it is crucial. Ise important than the mask your mask hygiene. When you wear a mask, you are less likely to wash her hands. The amount of people who leave the bathroom without washing their hands or rinsing their hands, dont wash their hands a have to wash her hands, lather, get in between your fingers. And forget about the back of your hands. We need the ability to test more people. We need masks. And again, we need hospital beds sick. En our patients get sen. Sanders and correctly if i am wrong, we need hand sanitizers that are affordable. I applaud the government of new york state on this. Companies saw the opportunity to make a lot of money, they raised their prices, and new york state, i think they will start producing them at the statewide level, which i think is exactly the right. Are nervous, are they not . they are concerned, i would not say they are nervous. We have made major demands of our employers, which is that they have plans and protocols in place using the precautionary principle that they clearly communicate directly to other nurses and Health Care Workers about their exposure to the. Ovid19 possible exposure that they provide education and training and not the just in time training, but actual training where you can practice it. And we also ask that they provide functioning negative pressure rooms and personal protections for nurses which right now are being kept under lock and key by managers. And the nurses have to go and get the key to get the mask, and they have been told during a code, hold your breath. I mean, it is startling that these things are happening. Surge ofned for a patients. One of our nurses that took care worked 80 patient hours in one week. That is obscene. That nursesake sure and other Health Care Workers that get put out of quarantine get a, and that they actually put those patients, those Health Care Workers, in quarantine. There was a nurse who was exposed and they said she could work because she was not symptomatic. The cruise ship gets better quarantine than Health Care Workers that are actually working with the virus. All of our federal government, we have asked for the highest levels of protection. We want enforcement that is mandatory. Right now, the c. D. C. Guidelines , all those recommendations are guidelines. They are not mandatory. So nurses cant, without the backing of the federal government and some state governments, actually make sure the state can enforce them. So they are merely suggestions which dont care for us. And we need more funding in an torgency spending package get these Public Health systems up and running, and functioning better than they are now. Sen. Sanders thanks. Anyone have anything to add . What this underscores is our reliance on china. Talking to a large group , a loting organization of these supplies come from china. Each up to about generic drugs, the most basic saying, rubber comes from china, rubber gloves that all doctors have to use. Growese pandemics start to in the future, we need to start looking at our infrastructure within the u. S. And see how we can look at making these things either here or with countries that are under free trade agreements that we agree with so that we are not so reliant on another country, because as these pandemics spread and orders start shutting down, we will be less and less prepared for doctors and other people like that at the front lines and the borders start shutting down. Sen. Sanders i wanted to open it up for any questions the media might have. Yes. [inaudible] sen. Sanders 15,000. Su at what point do you think it might be the right idea . Sen. Sanders that is a good question. Its something we have thought about a lot. We do not hold a rally without first conferring with local officials. Your question goes a lot more to a political rally, it goes to basketball games, theater events across this country. That is an issue that i think every organization, every sports team is going to have to look at. So to answer your very good question, it is an issue that we think about a whole lot. We do not go forward without consultation with local Public Health officials. Have you thought about, to masksmaybe providing are your event . We noticed you had purell stations. Sen. Sanders you heard dr. Dooley, it is not easy. She doesnt have enough masks for her office. Taking care of those patients, so [inaudible] masks, that would absolutely not be a good idea. It would not be a smart thing to do. We findtional masks should be immediately rerouted into the Health System, not a handed out wantonly in a public space. When masks are not used properly, studies have looked at this with regard to flu transmission, it can actually exacerbate transmission because people and up touching their face a lot more to take their mask. Ff and adjust their if they are used properly, they can be effective, but the general public using mosques could even be harmful masks could even be harmful. The c. D. C. Said today that people with Underlying Health conditions or over the age of 60 people over the age of 60 with Underlying Health conditions should take particular precautions. What precautions are you taking . Sen. Sanders well, i am surrounded by medical professionals. Thank you for asking. I am running for president of the United States and that requires a whole lot of work. There you go. Ok. [laughter] events,ur rallies and are you concerned that the coronavirus will have an impact on your campaign in terms of on theing, getting out trail and knocking on doors . Sen. Sanders obviously, everybody in any area of life in america is concerned about the coronavirus. If i am an manufacturer, if i am working in an office, what does that mean . Isry organization in america taking a hard look at what the coronavirus will mean to their operations, and yes, that is true about our campaign as well. Mucht want to get too into politics. Today, what i wanted to do is talk about what happened. Bring some knowledgeable people together in terms of how we go will the vaccine be so costly that some can afford it and some cannot . When i was growing up, we got the polio vaccine for free. Nobody asked how you pay for it. But they saw were children in iron lungs who were going to be damaged forever. To insane and cruel is it suggest that we have to figure out how to pay for it when we can actually go to war and not ask one question . But to prevent this kind of disease, we have to say, how can we pay for it . The cost of not paying for the vaccine is going to be orders of magnitude much more expensive. Do i approve of spending a few cents for a vaccine or spending thousands of dollars on hospital care . I think it makes a little bit more sense to invest a few cents and a vaccine. This is a nobrainer. That you would ask a question like that talks about one of the very Serious Problems facing this country. Does anybody in their right mind believe that if you are rich you shouldve waited afford a vaccine but if you are poor youre going to die . Is that really where we are at in the United States of america . We will make vaccines free to every man, woman, and child. There are people who are rationing insulin right now because we are paying 10 times more for insulin in this country than canada. I will tell the pharmaceutical industry, if you do not lower your prices, we will make generic drugs that are affordable to all people. One thing we have to remember, the value of the vaccine is not just in protecting the individual. It is in protecting everyone they come in contact with. Those fortune of 500 ceos who stocks just took a tumble today and said they would be willing to fund it. What was your reaction to the stock market decline . Sen. Sanders i think the Global Economy is really right now right now. Ht supply chains are being threatened. Workers are not going to work. This will impact economies all over the world. I think this is a difficult issue. It would certainly help if the world and the people of our own country had the confidence that the administration of the united is making decisions based on science.
